We are committed to supporting the NHS through winter pressures by helping to reduce the Delayed Transfers of Care (DTOC) rate that is causing unnecessary constraint on capacity, as well as considerable distress and unnecessary stays in hospitals for patients.

Our Short Term Reablement Service provides high-quality care for patients who are clinically optimised but cannot yet return home or to their care home setting. Working with you and your hospital discharge teams, we help to improve flow, create capacity and improve outcomes for patients. 

Our flexible intermediate care solutions

We support people of all ages and conditions through rehabilitation; whether that be short term reablement for patients to facilitate a safe and timely return to their home setting or step down care for individuals with complex needs ahead of finding specialist accommodation or enablement of their own homes.

We work with you to deliver a bespoke solution tailored to the patient, creating the best possible environment for their safe return home, recovery, future independence, and long-term care needs.

Short-term reablement: A patient-centric end-to-end intermediate care solution to enable your own service delivery under our CQC registration. Supplying nurses, support workers, health coordinators and managers, enabled with rostering, training and day to day management.

Step down care facilities: An intermediate care provision through the set-up of step-down care facilities for patients awaiting Pathway 1 and 2; working with you on the planning and delivery, stakeholder alignment, licencing, staffing, operating, facility management and clinical management and accountability.

Partnering with Xyla Elective Care, part of Acacium Group, we also have the expert digital capabilities to deliver remote monitoring of patients through our virtual wards service which has been designed and adapted for multiple care pathways; expediting hospital discharge and reducing the rate of both admissions and re-admissions for patients.
